Adopting a dog is up to a 15 year commitment, especially if a younger dog or puppy. If anything were to happen to the primary caregiver and they were unable to continue to care for the dog, what are your contingency plans to ensure that the pet continues to live in a loving home?

Are all members of your household aware of and supportive of the decision to adopt a dog?

Is there anything else you would like to share on your application, or that might be a consideration in ensuring that you have the best possible personality and lifestyle match to your needs? Have you applied to SULA in the past?
